The CORE Institute Specialty Hospital works with many insurance plans. If you have questions regarding your insurance coverage, or if you are a self-pay patient, please call our Benefit Verification Department at 602-795-6020 ext. 6055.

Our knowledgeable staff can assist you with your benefits and provide you with the expected out-of-pocket cost to the hospital. Our team can set up payment plans and answer basic questions regarding the insurance authorization and verification process for hospital services.

Please note, the hospital utilizes independent practitioners who are not employed by the hospital and bill independently, including your surgeon and anesthesiologist. In some cases, medical diagnostic services are required such as radiology and pathology services. You will receive separate billing statements from these physicians.
